Job Description:
SUMMARY
Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
-Provide personal care and assist with ADL's according to the Plan of Care – POC.
-Provide safe environment for patients.
-Safely ambulate/transfer patient as appropriate.
-Assist with passive and active ROM exercises as directed by the nurse.
-Prepare nutritional snacks as appropriate for the patient.
-Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
-Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
-Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
-Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
-Attend and actively contribute during patient care conference.
-Participate in HNA discipline meetings, team meetings, staff meetings, retreats and inservices as appropriate.
-Attend in-services to obtain 3 education hours per quarter.(need 12 a year)
-Participate in the on-call rotation as scheduled (Home Care only).
-Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
-Demonstrate positive customer-service responsiveness according to agency standards.
-Flexible in meeting patient/family needs that require variable shifts and weekends.
-Participate in chart review quality improvement activities and other meetings as assigned.
-Participate in coordination of and communication with volunteer staff.
-Provide basic caregiving education to caregiver/family members, such as turning and positioning, transfer techniques, and changing linens
-Complete annual competency requirements.
