SUMMARY
Provides nursing care is in accordance with the patient’s plan of care, including comprehensive assessment, monitoring of the health status, coordination of services, teaching and training activities, and management of the care plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES
-Start of Care Oasis Assessments
-Wound Care
-Medication management
-Catheter care
-Infusions
-Educate patients on health management and disease prevention.
-Document in Brightree by MatrixCare EMR system
