SUMMARY
Provides nursing care is in accordance with the patient’s plan of care, including: comprehensive assessment, monitoring of the health status, coordination of services, teaching and training activities, and management of the care plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
-Start of Care Oasis Assessments
-Wound Care
-Medication management
-Catheter care
-Infusions
-Educate patients on health management and disease prevention.
-Document in Brightree by MatrixCare EMR system
